Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by International Bible Church during the selected camp. In exchange for the acceptance of my child’s participation, I assume all risks and hazards incidental to the conduct of these activities and agree to release, absolve, and hold harmless International Bible Church, its officers, agents, volunteers, and representatives from any and all liability for injuries to my child arising out of travel to, participation in, or return from any and all camp sessions.

In the event of injury to my child, I hereby waive all claims against International Bible Church, including its leaders, volunteers, coaches, affiliates, participating individuals, sponsoring agencies, advertisers, and, if applicable, the owners and lessors of the premises used to conduct the event.

I understand that participation in physical activities, including but not limited to recreational games and sports such as basketball, involves certain inherent risks. These risks may include, but are not limited to, the potential for fractures, sprains, paralysis, or even death. I certify that I am aware of these risks and voluntarily assume full responsibility for any such risks arising from my child’s participation.

Medical Release and Authorization

As the parent and/or legal guardian of the named child, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional of my minor child in the event of a medical emergency which, in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the child’s life, risk of permanent physical disfigurement or impairment, or to alleviate undue pain, suffering, or discomfort if delayed.

Permission is hereby granted to the attending physician to proceed with any necessary medical or minor surgical treatment. In the case of an emergency arising from serious illness, major surgery, or significant injury, I understand that every reasonable effort will be made to contact me as quickly as possible. This authorization is granted only after a reasonable attempt has been made to reach me.

Furthermore, permission is granted to International Bible Church, including its staff, volunteers, directors, and team leaders, to provide or arrange for emergency care and first aid treatment as needed prior to the child’s admission to a medical facility.

This release shall remain in effect for the full duration of the registered event or camp session.

This release is executed voluntarily and with full understanding, for the sole purpose of authorizing emergency medical care in my absence, in order to protect the life and well-being of my child.
